When you enter your birthday you need to add all the digits including zeros if needed or it will not work properly. If your insurance is not on the list choose the closest one and we will still see if we can verify your insurance and get coverage for your visit after we get a copy of your insurance cards. Please let us know all insurances you are covered under, and if you are covered under another person, we will need their information to verify your insurance.
